Description

Our busy nephrology practice is seeking a dedicated Nurse Practitioner to join our team and work collaboratively with our providers to deliver high-quality, patient-centered care.

• Manage and treat patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD)

• Perform comprehensive patient assessments and physical examinations

• Develop, implement, and monitor individualized treatment plans

• Order and interpret diagnostic tests and laboratory studies

• Provide patient and family education regarding kidney disease, treatment options, and lifestyle modifications

• Collaborate with physicians and other healthcare professionals to ensure continuity of care

• Maintain accurate and timely medical documentation

• Participate in ongoing quality improvement and patient care initiatives

Requirements

Current Nurse Practitioner licensure (state-specific)
Experience in nephrology or chronic disease management preferred
Strong clinical, communication, and organizational skills
Ability to work effectively in a fast-paced clinical environment
Commitment to providing compassionate, evidence-based care
